The links below are to forms on the Virginia Judicial System website, an excellent site with information about Virginia Courts and the Virginia judicial system. For convenience, the links take you directly to the forms. The forms are in PDF format and you must use the Acrobat Reader program to view and use the forms. If you don't have Acrobat Reader on your computer, you can click here to download the program for free. The forms have been designed to allow you to fill out and print the forms on your printer. You cannot save the completed from to your computer. Once you close the Acrobat Reader program the data your entered will be lost and you must re-enter the data if you need to complete the form a second time. If you own the Adobe Acrobat program (which is not the reader program and requires purchase) you may be able to save the forms. Keep in mind that you must file the completed and printed form with the Commissioner of Accounts or the Court, depending on the form. Completing the form online does not file the form with the Court. When printing the completed form, use the "Print for Submission to Court" button at the top of the form and field highlighting will be removed. If you print the form using the print button on your browser, the fields may remain highlighted (grayed out) when printed. Waiver
of Qualification. This form is used to evidence the decision not to
serve as a personal representative by a person appointed in the will to

Information Form. Many Clerks will complete this form on the computer
at time of probate and/or qualification. You may want to complete this
form in advance to know all information that must be provided. Probate
Tax Return. This form will be completed at time of probate and/or
qualification based on an estimate of the total value of the personal
and real property assets of the estate. Trust
Information Form. Like the Probate Information form, you should have
the information required in this form available when seeking qualification
as a trustee for a testamentary trust. Inventory
